What do I do if I have mislaid or damaged my stamp?

If your stamp has been mislaid or damaged complete the OV45 form and return it to the APHA OV Team.

If your stamp has been stolen, you will need to contact your local Police Station for a Crime Reference Number before contacting the OV Team. A Lost Property Number is required for a mislaid stamp. This information will be required before a replacement OV stamp can be issued.

On receipt of this information the OV Team (APHA) will arrange for a replacement stamp to be issued within 10 working days.

What do I need to do if I'm setting up a new Practice?

Download and complete the following new practice set up forms:

  • Details of Suppliers A/AP201,
  • General procedures for Official Veterinarians, Her Majesty’s customs and excise conditions on OV fees self-billing invoicing system - OV10
  • General procedures for official veterinarians, payment of OV fees OV09
  •  Confidentiality Agreement.

If your Practice will be TB testing please complete the Sam Declaration form OVb37. Please return these forms to APHA by post.

On receipt of the completed documents your local APHA office will be notified and may wish to inspect your Practice before the set up process is completed. On receipt of confirmation that your Practice can be set up, from your local APHA office, APHA’s OV Team will complete the Practice set up process and notify you.

On completion of the Practice set up, the OV Team will ensure that all OV records, for that Practice, are created/updated accordingly and the relevant documentation issued.

What rates are payable for TB testing carried out, by OVs?

Government funded TB testing in England and Wales is delivered via contracted Veterinary Delivery Partners and no payments for TB testing are made directly to OVs or practices from APHA.

There is a scale of Fees for Services Rendered in Scotland by Official Veterinarians found in OV19(S).

What do I do if my name changes during my appointment?

You will need to update your improve international account information before any action can be taken to update your Sam profile. Complete the OV57 form and send to the OV Team (APHA) who will then process the information you have submitted and will update your record within 10 working days. Copies of certificates updated with name changes can be found on Improve International.

What do I need to do if I transfer to another Practice?

You will need to update your improve international account information before any action can be taken to update your Sam profile. To request a Sam update complete the OV58 practice form and send to the OV Team (APHA) who will process the information you have submitted and your Sam record will be updated within 10 working days of submission. Updated certificates can be found on Improve International.

What do I do if I don't meet the bovine TB testing criteria for Tuberculin Testing revalidation?

If you think you don't meet the bovine TB testing criteria for Tuberculin Testing revalidation or need an extension in order to meet the testing requirements to revalidate, see the http://apha.defra.gov.uk/official-vets/training-and-authorisation/index.htm instructions.

How do I register on Sam, or get access for administrative staff?

Return the OV59 form to the OV team (APHA) who will process the information you have submitted within 10 working days. A known-facts letter will be sent out with log-on details to the practice address.

What to do if I leave my current practice and my OV status is no longer required

Complete the OV55 form and send to the OV Team (APHA) who will process the informaiton you have submitted within 10 working days.
