3. When the animal(s) have arrived at the home isolation premises, the VDP Vet must arrange for a weekly follow up call or visit for the duration of the animal(s) home isolation period. The call or visit should be recorded using the Supervising Veterinarians Weekly Checklist for Pet Animal Home Isolation (ID127). VDP Vets should email the completed Supervising Veterinarians Weekly Checklist for Pet Animal Home Isolation (ID127) to the Quarantine and Escalations Team: ukraine.quarantineandescalations@apha.gov.uk by exception where an issue with the home isolation premises or the animal(s) that needs to be escalated to APHA.

4. In the final seven days of home isolation the VDP Vet should complete the Supervising Veterinarians Final Checklist for Pet Animal Home Isolation (ID142). VDP Vets should email the completed Supervising Veterinarians Weekly Checklist for Pet Animal Home Isolation (ID127) to the Release Team: ukraine.releases@apha.gov.uk.
